RN Case Manager
The registered nurse is in charge of planning, organizing and directing hospice care. This person has experience in nursing as well as community health education. In a job as an RN, you’ll use community resources to plan and direct services that meet the needs of patients and their families, whether in their homes or in their communities.
LPN CC, PRN
The licensed practical/vocational nurse works under the supervision of an RN to provide care directly to the patient. In this job, you will be responsible for following the plan of care and administering treatments, as well as collaborating with the Capital City Hospice care team to provide a positive care experience.
STNA Certified Hospice Aide
The STNA/Hospice Aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient’s personal needs and to promote comfort. The STNA/Hospice Aide is responsible for observing the patient, reporting these observations and documenting observations and care performed. The STNA/Hospice Aide will be assigned in a manner that promotes quality, continuity, and safety of a patient’s care.

Our Services
