Anyone can refer a resident to hospice!

Family members, physicians, friends, nursing facility staff—even members of the patient’s clergy—can make referrals.

Once a referral is received, our Hospice Admissions Staff will contact the patient’s primary physician to ascertain if the patient is appropriate for hospice care. Medical records will be reviewed and the hospice staff will then visit with the patient and family to explain hospice services and to ascertain patient and family wishes.

Our goal is to admit patients to hospice care within 24 hours of a completed referral.

Dear friends at Capital City Hospice:

“A very humble thank you for the superior and compassionate care your organization gave to my mother during the last month of her life. The nurses that visited were always calm and caring not only to my mom but also calming to me as well. The floral arrangement was above and beyond and enjoyed by all. We are forever grateful and will always sing your praises.”

“I am writing to let you know, even though words are inadequate, how much my family appreciates your presence and support and professional care for my mother. Your team connected easily with our family and gave immeasurable support. Capital City Hospice made my mother’s passing easier for her and our entire family. We will always remember your kindness!”
