Anyone can refer a resident to hospice!
Family members, physicians, friends, nursing facility staff—even members of the patient’s clergy—can make referrals.
Once a referral is received, our Hospice Admissions Staff will contact the patient’s primary physician to ascertain if the patient is appropriate for hospice care. Medical records will be reviewed and the hospice staff will then visit with the patient and family to explain hospice services and to ascertain patient and family wishes.
Our goal is to admit patients to hospice care within 24 hours of a completed referral.
